This must surely have involved some problem in the control room with the mixing desk. I can't believe that live TV shows rely entirely on the presenters remembering to turn their mics on and off. If they did, this sort of occurence would be far more common. More likely is that there's a sound engineer who brings the different mic sources in and out of the feed. Something must have gone wrong with this process.
